An offer for older people in Mainz

Most people want to live independently in their own apartment or house and familiar surroundings for as long as possible. The Community Nurse Plus program is designed to support seniors in particular.
This free, preventive service is aimed at older people who do not yet need care but would like supportive advice at this stage of their lives.
The aim is to maintain the independence of elderly people for as long as possible and to avoid or delay the need for care through targeted interventions. If the need for care arises, the Community Nurse Plus specialists refer the patient to their colleagues at the care support centers and nursing services.
The specialist community nurses themselves do not perform any nursing tasks.

The Community Nurse Plus program is offered throughout the city of Mainz and is funded by the Rhineland-Palatinate Ministry of Labor, Social Affairs, Transformation, and Digitalization, supplemented by municipal funds.

Tasks: The community nurse plus specialist takes time and listens.

The qualified community nurses make home visits, listen to the concerns and wishes of older people in personal conversations, and advise them on issues such as coping with everyday life, adapting their living space, mobility, or loneliness. They provide information about opportunities to meet other people, exercise programs, or cultural events. They build supportive bridges to the respective services, thus facilitating access.
Their experiences from conversations with older people are also used to initiate new services or further develop existing ones.
