The severely disabled ID card is a nationally standardized document certifying that the holder is a severely disabled person. It specifies the degree of disability (GdB) and any additional marks. It also states the period of validity.
To obtain a severely disabled ID card, you must submit an application for determination of the degree of disability. The Office for Social Affairs of the State of Rhineland-Palatinate is responsible for issuing the card. The city administration of Mainz cannot issue it.

Before a severely disabled person's pass can be issued, an assessment procedure must first be carried out. This assessment procedure is carried out by the Social Affairs Office (Versorgungsamt) responsible for the applicant's place of residence.
