The Severe Disability Card is a nationally standardized document certifying that the holder is a person with a severe disability. It specifies the degree of disability (GdB) and any additional designations. It also indicates the period of validity.
To obtain a Severe Disability Card, you must submit an application for the determination of the degree of disability. The State Office for Social Affairs, Youth, and Welfare is responsible for issuing the card. The City of Mainz cannot issue it.
Before a severely disabled person’s ID card can be issued, an assessment procedure must first be carried out. This assessment procedure is conducted by the Office of Social Affairs (Versorgungsamt) responsible for the applicant’s place of residence.
